Important Info For 6th & 11th Grade Parents

ATTENTION: Parents of 11th Grade Students

 

All 11th grade student needs to receive the meningococcal conjugate vaccine (MCV4) before entering the 12th grade.  This is a requirement of the Ohio Department of Health. If your student received the first dose of the meningitis vaccine before they were 16, a second dose is required.  If your student received a dose of MCV4 on or after their 16th birthday, they meet the requirement for school and do not need another dose.

The immunization can be obtained at your physician’s office or at either location of the Williams County Health Department. Please call ahead to make an appointment. Medicaid and private health insurances are accepted, and no one is denied service for inability to pay.

After receiving the MCV4 vaccine please send a copy of the updated immunization record to the school office. For questions or concerns call 419-633-6233.
